LiveWell Kershaw Coalition Clear Impact Finalist

LiveWell Kershaw Coalition Clear Impact Finalist

This year, Clear Impact launched the inaugural Racial Equity Turn the Curve Plan Poster Contest – a contest aimed at helping agencies and organizations learn how to effectively use “Turn the Curve” thinking to address racial equity issues.

We are thrilled to announce that our work was selected as one of five finalists across the nation! We presented our Turn the Curve Plan at Clear Impact's Measurable Equity 2021 conference on Tuesday, November 9th at 3pm EST. Conference attendees voted for the winner. The winner of the plan received $25,000 to advance their work! LWK is honored to have represented South Carolina in this competition. Our submission reflects the uncovering of elevated A1c levels among Hispanic patients with diabetes at the Community Medical Clinic of Kershaw County, what the clinic is doing, and what coalition members can do to address this health disparity.

About: LWK was founded by a group of community members interested in using data to guide action aimed at addressing the health issues faced by the residents of Kershaw County. Today, LWK works to foster collaboration between citizens of all ages, working in partnership with public and private sector organizations and schools to increase community awareness of health issues.

Contest Submission: “A1c Level Across ALL Patients with Diabetes”
